CPET (Cardiopulmonary Exercise Testing)

A graded exercise test measuring combined heart, lung, and metabolic function under increasing physical load, used to guide individualized pacing.

What it is & how it works

A graded exercise test — typically on a treadmill or cycle ergometer — that measures combined heart, lung, and metabolic function under increasing physical load, including VO2 max and ventilatory threshold, via a metabolic cart and continuous ECG monitoring.

How it's done

Conducted under direct supervision by an exercise physiologist or physician, typically lasting 20–30 minutes including warm-up and recovery, with the patient wearing a breathing mask/mouthpiece, ECG leads, and a blood-pressure cuff. Contraindications are screened for before the test begins.

Yes — typically no food or drink other than water for about 4 hours before, no caffeine for 24 hours, and no smoking or vaping for 8 hours, along with comfortable exercise clothing and footwear. Your care team will confirm any medication adjustments in advance.

CPET is a well-established, closely supervised test with a long safety record; the most common effects are breathlessness and muscle fatigue from the exertion itself, and serious complications are rare. It isn't performed in people with certain acute conditions — such as a recent heart attack, uncontrolled severe hypertension, unstable heart valve disease, or advanced pregnancy — and contraindications are screened for beforehand.
